Ad fatigue is what happens when an ad that used to work stops working, not because you changed anything, but because the same people have seen it so many times that they have stopped reacting to it.

You will usually notice it as a set of numbers moving together rather than one bad day. Frequency keeps climbing as the same audience sees the ad again and again, the click-through rate slides because people who have already scrolled past it a dozen times no longer stop for it, and your cost per result and your CPM drift upward because Meta has to work harder, and pay more, to keep putting a tiring ad in front of an audience that is done with it. The creative is exactly the same as the day it was winning. The audience just got bored.

This matters because fatigue is one of the quietest ways a good campaign falls apart. When the results start slipping, the natural reaction is to decide the ad has stopped converting and go digging through the settings, but if you are watching frequency go up while CTR goes down, you are almost certainly looking at fatigue rather than a broken funnel, and no amount of setting changes will fix a room full of people who have already seen your ad.

The fix is almost always fresh creative, not fresh targeting or a bigger budget. Give the audience something genuinely new to look at, a different hook, a different format, a real new ad, rather than pausing and restarting the same tired one and expecting a different outcome. And keep in mind that on a small audience fatigue shows up faster, because there are fewer people to spread your impressions across, so when your pool is narrow you should plan to refresh sooner than you think.

When frequency is rising and everything else is sagging, the ad is not broken, it is just old, so give them something new to see.
